How you place the foam is going to depend on exactly how the helm is constructed. Put it on your head with no padding and see how it wiggles around.
Is your head's circumference too small for the interior circumference of the helm? Put some padding around the band.
Sit too low on your head? Put some padding in the top.
Once it fits on your head securely, start thinking about getting hit in the head with a melee weapon. Like Kraesh said - if there are ear pieces, think about how it'd feel if you got smoked there. Add some foam if you think it'll hurt.
Have a big dangly lobster tail? Maybe add some padding under that to save your neck some pain.
It's really going to boil down to your individual helmet and how it currently fits on your head vs. how you want it to fit.
