Is the pommel counted as part of the 18" or 1/3 rule for handles, or is the handle length prescribed specifically for the area between the pommel and the blade/crossguard/ricasso area?

Pommels are not included in the handle. The handle is the unpadded area. For measurement purposes, pommels can be considered incidental padding.
