I went to the Notary republic today, and she said that only the first page would be needed. Wanted to double check just in case. Somebody who has done this before fill me in. Ty!

Ty for the reply. My question is, does it matter if the last page has notary seals too? or is the first page enough?

The waiver is the important part. The second page is for emergency medical stuff and doesn't need to be notarized, though it should be for safety.
